Hair is one of the most important items of the psycho-social skin function. Its quantity and quality underlines the final image of the person and the majority of people recognize it as a symbol of attractiveness or non-attractiveness.

Hair loss is one of the most uncomfortable disorder for patients both women and men, which is connected with a psycho-social trauma.

These generally well known facts led us to the creation of a new outpatient clinic dealing with hair and scalp diseases. The target population will consist of patients with scalp disorders like dandruff, psoriasis, seborrheic or atopic dermatitis. We shall also focus on patients suffering from enhanced hair loss or reduced hair quality. Enhanced hair loss can be caused by drug intake or during unbalanced alimentary habits. It is often associated with diseases called androgenetic alopecia in women and men, alopecia areata or scarring alopecia.

Exact diagnostics made by an expert is a prerequisite key to successful treatment.

The principle of digital dermatoscopy is early detection of incipient melanoma (the most malignat skin tumor) in stage when it is still easily curable - if this tumor is surgicaly treated in initial phase - e.g. when all its cells are still in original site ( means in skin), curability is in fact 100%.

The timing of such examination in autumn (after massive summer sun exposure) is more than proper - it has been proved the sun has promoting efect on development of melanoma.

I přes to ale nadále stoupá i výskyt maligního melanomu. Je to nepřípustné, aby v dnešní době odešel pacient z ambulance dermatologa se suspektním maligním melanomem na své kůží jen proto, že kožní lékař nerespektoval zásady správného vyšetřovaní pigmentových névů. V tomto boji nám výrazně pomáhá digitalní dermatoskopie, neinvasivní diagnostická metoda, která používá zvláštním způsobem upravenou osvětlenou lupu a zvyšuje přesnost diagnózy o 10-27 % v porovnání s klinickým vyšetřením.

DERMATOSKOPIE

Dermatoskopie je neinvasivní diagnostická metoda, která používá zvláštním způsobem upravenou osvětlenou lupu. Rozdíl spočívá v tom, že na spodní části lupy je ještě jedno průhledné sklo, které se přímo dotýká při vyšetření kůže. Navíc, štěrbina mezi kůží a tímto sklem je vyplněna tekutinou. Toto vlhké prostředí vyplní veškeré kožní nerovnosti pod sklem, čímž se zruší běžný odraz světla na kožním povrchu, a to umožní posuzovat struktury v epidermis, dermoepidermální junkci a v papilární dermis. Tato metoda zvyšuje správnou klinickou diagnózou melanocytárních, nemelanocytárních, zhoubných i nezhoubných kožních projevů. U maligního melanomu zvyšuje přesnost diagnózy o 10-27 % v porovnání s klinickým vyšetřením. Doposud byla dermatoskopie nejvíce využívána k diferenciální diagnostice pigmentových projevů. V současnosti se objevují sdělení, která popisuji dermatoskopii jako metodu, která nám může pomoci i v diferenciální diagnostice např. psoriasis vulgaris vs. lichen ruber nebo psoriasis vulgaris vs. dermatitis seborrhoica. Významnou roli hraje dermatoskopie v odlišení subunguálního hematomu od pigmentované onychomykózy, kdy se na místo totální nebo parciální ablace nehtu nasadí antimykotická léčba.
